Wordpress Custom Theme designer needed for Therapy company

I started my Occupational Therapy company a few months ago and I require someone to create the look and feel for the website.  The content has been completed and I only require a Wordpress Custom Theme designer to setup the website's look and feel.

I like designs that are clean and sleek and very web 2.0.  I like white space.  With that being said, the front page should have rotational pictures for the three services that I provide (Wheelchair assessments, Ergonomic assessments and Bio-mechanics assessments).

There should be 6 tabs in place and a link on the logo to take the user back to the home page.
