PHP Programmer - ASAP

I need the code for a PHP form (simple form with contact details, subject, etc.), so I can just paste it into my clients website. The main thing it has to do is have some way of preventing spam (be it with images, or some other way). My problem: I need it for this afternoon, asap! My budget: 40$ Send me an email with your contact, fees, and some links to you previous coding work.
