Simple Php Form : Image, Title, Descp, and Contact Info

Hello, This project is really straight forward. I would like to find someone to pass these small quick projects to. We have an existing site that needs a page setup with a temporary solution. The function is a basic php form that will post an image, title, description, and contact info. Admin is simple : ADD, EDIT, and DELETE.
